Individual Assessment for Residency Training

The following questions are intended for the incoming resident to conduct a self-assessment prior to beginning their residency year. The responses assist in planning experiences for the resident in the upcoming year. Please answer the following questions.

1.      State your career goals both short term (three to five years) and long range (ten years).

2.      Describe your current practice interests.

3.      What do you consider your strengths, and how do you plan to continue to build upon these strengths?  This should include your professional and personal strengths.

4.      What do you consider your weaknesses, and how would you like to improve these through your residency training?

5.      Given your listed career goals, interests, strengths, and weaknesses, list at least three (3) goals that you wish to accomplish during your residency.

6.      In order to develop an individual plan for your residency training, what areas of training would you like to concentrate on during your residency program? (List in order of importance)

7.      Describe the frequency and type of preceptor interaction you feel to be ideal. Where do you see the preceptor fitting into your professional development and maturity?

8.      What is your personal strategy for life-long continuing education?

9.      How do you rank yourself in the following areas? (1=highest ranking, 5=lowest ranking)

a.     Written communication skills
b.     Verbal communication skills
c.     Public speaking
d.     Interpersonal skills
e.     Therapeutics        
f.      Understanding of hospital pharmacy practice
g.     Follow-through
